100 Stories was a two-year co-produced project bringing together young people, parents and carers, professionals and partners across North Wales to explore the experiences of young people becoming adults and moving through the health, education and social care system.
The project brought together young adults, parents and carers, and professionals from across North Wales, who shared their experiences of neurodevelopmental, learning disability and adolescent mental health services.
Their stories included frustration, uncertainty and feeling unheard, but also resilience, determination and hope. Together, they show why transition is not something that simply happens on a young person’s eighteenth birthday. For many young people, it is a much longer journey that can be impacted by childhood experiences and wider circumstances and needs continuity, understanding and support.
100 Stories is about more than collecting experiences. Through storytelling, participants are supported to build confidence, leadership skills and to express what mattered to them, with the intention of building the conditions and evidence base to change the system for future generations.
The 100 Stories Report
The report beings together the learning and evidence gathered throughout the project. It highlights key issues, the impact of storytelling and the actions needed to create a more joined up, compassionate approach to transition and navigating services across the health, education and social care system.
